| Clicking any of the Drop Lists will display a wide variety of choices. |
| Clicking on the Specific Activities list under Activities of Daily Living will display the following list. |
| The User may select any choice from the "Specific Activities" drop down list and then add the Assistance Required, then repeat the procedure for all applicable choices of activities. |
| Clicking
OK will save your changes and take you back to the previous screen. Clicking Cancel will will cancel your changes and take you back to the previous screen. Click This to Proceed to the Next Screen. |
